Variant summary
Our verdict is Likely pathogenic. The variant received 9 ACMG points: 10P and 1B. PVS1PM2BP6
The NM_058195.4(CDKN2A):c.253C>T(p.Gln85*) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000131 in 152,132 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. Q85Q) has been classified as Likely pathogenic. Variant results in nonsense mediated mRNA decay.

The c.210C>T variant (also known as p.P70P), located in coding exon 2 of the CDKN2A gene, results from a C to T substitution at nucleotide position 210 and does not change the amino acid at position 70 of the p16 isoform. Of note, this variant is also known as p.Q85* (c.253C>T) in the p14(ARF) isoform and results from a C to T substitution at nucleotide position 253. The evidence supporting a relationship between p14(ARF) and melanoma-pancreatic cancer syndrome is limited; therefore, the association of this variant with this gene-disease relationship is unknown. However, the association of this variant in the p16 isoform with melanoma-pancreatic cancer syndrome is unlikely. -

Reported using an alternate transcript of the gene; Nonsense variant predicted to result in protein truncation, although loss-of-function variants have not been reported downstream of this position in the protein; Observed in an individual with squamous cell carcinoma of the head and neck (Lang et al, 2002); Not observed in large population cohorts (gnomAD); This variant in the p14-ARF isoform also results in a likely benign variant in the p16 protein, p.Pro70=; This variant is associated with the following publications: (PMID: 25925381, 12118338, 27535533, 31970404) -
