The c.2257T>C (p.W753R) alteration is located in exon 10 (coding exon 10) of the ESPN gene. This alteration results from a T to C substitution at nucleotide position 2257, causing the tryptophan (W) at amino acid position 753 to be replaced by an arginine (R). This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This variant has been identified in the homozygous state in individual(s) with features consistent with ESPN-related deafness (Alkhidir, 2024). This amino acid position is highly conserved in available vertebrate species. This alteration is predicted to be deleterious by in silico analysis. Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -

The W753R variant in the ESPN gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The W753R variant is not observed in large population cohorts (Lek et al., 2016). The W753R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. We interpret W753R as a variant of uncertain significance. -
