Variant summary
Our verdict is Uncertain significance. The variant received 4 ACMG points: 4P and 0B. PM2PP3_Moderate
The NM_001163435.3(TBCK):c.1532G>A(p.Arg511His) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.000000685 in 1,459,022 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 14/22 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★). Synonymous variant affecting the same amino acid position (i.e. R511R) has been classified as Likely benign.

The p.Arg511His variant in TBCK has been reported, in the homozygous state, in one individual with TBCK-related intellectual disability syndrome (PMID: 27040692), and has been identified in 0.001% (1/90828) of South Asian chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP ID: rs869320711). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 225237) and has been interpreted as likely pathogenic/pathogenic by University of Washington Center for Mendelian Genomics (University of Washington) and OMIM. Computational prediction tools and conservation analyses suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. The p.Arg511His variant is located in a region of TBCK that is essential to protein folding and stability, suggesting that this variant is in a functional domain and slightly supports pathogenicity (PMID: 27040692). In summary, while there is some suspicion for a pathogenic role, the clinical significance of this variant is uncertain.
