Genes affected
ACADM (HGNC:89): (acyl-CoA dehydrogenase medium chain) This gene encodes the medium-chain specific (C4 to C12 straight chain) acyl-Coenzyme A dehydrogenase. The homotetramer enzyme catalyzes the initial step of the mitochondrial fatty acid beta-oxidation pathway. Defects in this gene cause medium-chain acyl-CoA dehydrogenase deficiency, a disease characterized by hepatic dysfunction, fasting hypoglycemia, and encephalopathy, which can result in infantile death. Alternatively spliced transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Jul 2008]

ACMG classification
Classification made for transcript
Verdict is Pathogenic. Variant got 14 ACMG points.
PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most 50 bp of the penultimate exon, not predicted to undergo nonsense mediated mRNA decay. There are 13 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 1-75761364-C-CT is Pathogenic according to our data. Variant chr1-75761364-C-CT is described in ClinVar as [Pathogenic]. Clinvar id is 226109.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Pathogenic, no assertion criteria provided | clinical testing | Natera, Inc. | Nov 27, 2017 | - - |
Pathogenic, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | Dec 09, 2022 | Variant summary: ACADM c.1189dupT (p.Tyr397LeufsX5) results in a premature termination codon, predicted to cause a truncation of the encoded protein which disrupts the last 25 amino acids, and includes part of the C-terminal domain (IPR009075). At least one other frameshift variant downstream of this position has been classified as pathogenic in ClinVar (Variation ID: 226069). The variant allele was found at a frequency of 4e-06 in 250084 control chromosomes (gnomAD). c.1189dupT has been reported in the literature in the compound heterozygous state together with the common pathogenic variant, c.985A>G, in at least two individuals affected with Medium Chain Acyl-CoA Dehydrogenase Deficiency (e.g. Andersen_1997, Couce_2013, Ventura_2014). These data indicate that the variant is likely associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. Both classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. - |
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Oct 31, 2023 | This sequence change creates a premature translational stop signal (p.Tyr397Leufs*5) in the ACADM gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 25 amino acid(s) of the ACADM protein. This variant is present in population databases (rs770378052, gnomAD 0.007%). This premature translational stop signal has been observed in individual(s) with MCAD deficiency (PMID: 9158144; Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 226109). For these reasons, this variant has been classified as Pathogenic. - |
