The homotetramer enzyme catalyzes the initial step of the mitochondrial fatty acid beta-oxidation pathway. Defects in this gene cause medium-chain acyl-CoA dehydrogenase deficiency, a disease characterized by hepatic dysfunction, fasting hypoglycemia, and encephalopathy, which can result in infantile death. At least one other frameshift variant downstream of this position has been classified as pathogenic in ClinVar (Variation ID: 226069). The variant allele was found at a frequency of 4e-06 in 250084 control chromosomes (gnomAD). c.1189dupT has been reported in the literature in the compound heterozygous state together with the common pathogenic variant, c.985A>G, in at least two individuals affected with Medium Chain Acyl-CoA Dehydrogenase Deficiency (e.g. Andersen_1997, Couce_2013, Ventura_2014). These data indicate that the variant is likely associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. Both classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
