MLH1 (HGNC:7127): (mutL homolog 1) The protein encoded by this gene can heterodimerize with mismatch repair endonuclease PMS2 to form MutL alpha, part of the DNA mismatch repair system. When MutL alpha is bound by MutS beta and some accessory proteins, the PMS2 subunit of MutL alpha introduces a single-strand break near DNA mismatches, providing an entry point for exonuclease degradation. The encoded protein is also involved in DNA damage signaling and can heterodimerize with DNA mismatch repair protein MLH3 to form MutL gamma, which is involved in meiosis. This gene was identified as a locus frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). [provided by RefSeq, Aug 2017]

This variant disrupts the p.Glu37 amino acid residue in MLH1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 12624141, 18415027, 20020535, 22753075, 27435373, 31815095; Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

The glutamic acid at codon 37 is replaced by glycine, an amino acid with similar properties. This variant has been reported in individuals meeting Amsterdam criteria for Lynch syndrome; some also have Lynch tumors demonstrating loss of MLH1 on immunohistochemistry and high microsatellite instability (de Rosa N et al. J Clin Oncol, 2016 09;34:3039-46; Ambry internal data). Another alteration at the same codon, p.E37Q (c.109G>C), has been detected in an individual meeting Amsterdam criteria for Lynch syndrome (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 250626 control chromosomes in GnomAD. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.110A>G has been reported in the literature in individuals affected with Lynch Syndrome without strong evidence for causality (Pigatto_2004 and de Rosa_2016). These reports do not provide unequivocal conclusions about association of the variant with Lynch Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two same codon and different amino acid change variants (c.109G>C/p.E37Q and c.109G>A/p.E37K) are reported as DM in HGMD and interpreted in ClinVar (Variation ID 231565 and 89640, respectively). However, the pathogenicity of those variants cannot be ascertained by our standards. Both E37Q and E37K do not have enough clinical cases, although E37K may result in reduced MMR activity in vitro (~10%). Furthermore, E37Q has been seen in cis with a likely null allele c.65_67delinsTA in a patient with Rectal Cancer (not in ClinVar, DM in HGMD, reported in Bashyam_2014 ). The following publications have been ascertained in the context of this evaluation (PMID: 20233461, 27432916). Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation and inconsistent interpretations have been reported (Pathogenic, Likely pathogenic and VUS, respectively). Based on the evidence outlined above, the variant was classified as uncertain significance. -
