Variant summary

Our verdict is Pathogenic. The variant received 15 ACMG points: 15P and 0B. PS1_ModeratePS3PM1PM2PM5PP3_ModeratePP5

The NM_001048174.2(MUTYH):c.763A>T(p.Met255Leu) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). ClinVar reports functional evidence for this variant: "SCV003867814: "In a massively parallel cell-based functional assay testing 7,8-dihydro-8-oxoguanine:adenine (8OG:A) repair activity, a byproduct of oxidative damage, this variant was reported to be non-functional (Hemker SL et al. Am J Hum Genet. Published online July 29, 2025. DOI: 10.1016/j.ajhg.2025.07.005)."". Another nucleotide change resulting in the same amino acid substitution has been previously reported as Likely pathogenic in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. M255T) has been classified as Likely pathogenic.

Genes affected
MUTYH (HGNC:7527): (mutY DNA glycosylase) This gene encodes a DNA glycosylase involved in oxidative DNA damage repair. The enzyme excises adenine bases from the DNA backbone at sites where adenine is inappropriately paired with guanine, cytosine, or 8-oxo-7,8-dihydroguanine, a major oxidatively damaged DNA lesion. The protein is localized to the nucleus and mitochondria. This gene product is thought to play a role in signaling apoptosis by the introduction of single-strand breaks following oxidative damage. Mutations in this gene result in heritable predisposition to colorectal cancer, termed MUTYH-associated polyposis (MAP). Multiple transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Apr 2017]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 15 ACMG points.

PS1
Transcript NM_001048174.2 (MUTYH) is affected with MISSENSE_VARIANT having same AA change as one Pathogenic present in ClinVar.
PS3
PS3 evidence extracted from ClinVar submissions: SCV003867814: "In a massively parallel cell-based functional assay testing 7,8-dihydro-8-oxoguanine:adenine (8OG:A) repair activity, a byproduct of oxidative damage, this variant was reported to be non-functional (Hemker SL et al. Am J Hum Genet. Published online July 29, 2025. DOI: 10.1016/j.ajhg.2025.07.005)."
PM1
In a hotspot region, there are 9 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 5 benign, 31 uncertain in NM_001048174.2
PM2
Very rare variant in population databases, with high coverage;
PM5
Other missense variant is known to change same aminoacid residue: Variant chr1-45332251-A-G is described in ClinVar as Conflicting_classifications_of_pathogenicity. ClinVar VariationId is 406833.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.932
PP5
Variant 1-45332252-T-A is Pathogenic according to our data. Variant chr1-45332252-T-A is described in ClinVar as Conflicting_classifications_of_pathogenicity. ClinVar VariationId is 1486820.
