Genes affected
FH (HGNC:3700): (fumarate hydratase) The protein encoded by this gene is an enzymatic component of the tricarboxylic acid (TCA) cycle, or Krebs cycle, and catalyzes the formation of L-malate from fumarate. It exists in both a cytosolic form and an N-terminal extended form, differing only in the translation start site used. The N-terminal extended form is targeted to the mitochondrion, where the removal of the extension generates the same form as in the cytoplasm. It is similar to some thermostable class II fumarases and functions as a homotetramer. Mutations in this gene can cause fumarase deficiency and lead to progressive encephalopathy. Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Apr 06, 2023 | For these reasons, this variant has been classified as Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt FH protein function. ClinVar contains an entry for this variant (Variation ID: 237116). This variant is also known as 349A>G (R117G). This missense change has been observed in individuals with hereditary leiomyomatosis (PMID: 15937070; Invitae). It has also been observed to segregate with disease in related individuals. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces arginine, which is basic and polar, with glycine, which is neutral and non-polar, at codon 160 of the FH protein (p.Arg160Gly). - |
Hereditary cancer-predisposing syndrome Pathogenic:1
Pathogenic, criteria provided, single submitter | clinical testing | Ambry Genetics | Nov 16, 2023 | The p.R160G pathogenic mutation (also known as c.478A>G), located in coding exon 4 of the FH gene, results from an A to G substitution at nucleotide position 478. The arginine at codon 160 is replaced by glycine, an amino acid with dissimilar properties. This alteration, sometimes designated as c.349A>G in the literature, has been identified in individuals with a personal and/or family history that is consistent with FH-related disease (Ambry internal data; Wei MH et al. J. Med. Genet., 2006 Jan;43:18-27; Picaud S et al. J. Inherit. Metab. Dis., 2011 Jun;34:671-6; Scharnitz T, et al. Am J Cancer Res 2023 Jan;13(1):236-244). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation. - |
