Hereditary cancer-predisposing syndrome Uncertain:2
The p.P399Q variant (also known as c.1196C>A), located in coding exon 9 of the SDHA gene, results from a C to A substitution at nucleotide position 1196. The proline at codon 399 is replaced by glutamine, an amino acid with similar properties. This alteration was identified in an individual diagnosed with breast cancer (McDonald JT et al. PLoS One, 2022 Oct;17:e0273835). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Paragangliomas 5;C5700310:Mitochondrial complex II deficiency, nuclear type 1 Uncertain:1
This sequence change replaces proline, which is neutral and non-polar, with glutamine, which is neutral and polar, at codon 399 of the SDHA protein (p.Pro399Gln). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with SDHA-related conditions. ClinVar contains an entry for this variant (Variation ID: 480869). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on SDHA protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
not provided Uncertain:1
The P399Q variant in the SDHA gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The P399Q variant is not observed in large population cohorts (Lek et al., 2016). The P399Q variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. In-silico analyses, including protein predictors and evolutionary conservation, support a deleterious effect. We interpret P399Q as a variant of uncertain significance. -
