Variant summary
Our verdict is Likely pathogenic. Variant got 8 ACMG points: 8P and 0B. PM1PM2PP2PP3_ModeratePP5
The NM_170707.4(LMNA):c.1090G>A(p.Asp364Asn) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. D364D) has been classified as Likely benign.

Charcot-Marie-Tooth disease type 2 Pathogenic:1
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Nov 28, 2022 | This missense change has been observed in individual(s) with clinical features of limb girdle muscular dystrophy and/or Emery-Dreifuss muscular dystrophy (PMID: 27363342, 32571898). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces aspartic acid, which is acidic and polar, with asparagine, which is neutral and polar, at codon 364 of the LMNA protein (p.Asp364Asn). ClinVar contains an entry for this variant (Variation ID: 242000). For these reasons, this variant has been classified as Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt LMNA protein function. - |

Cardiovascular phenotype Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Nov 02, 2022 | The p.D364N variant (also known as c.1090G>A), located in coding exon 6 of the LMNA gene, results from a G to A substitution at nucleotide position 1090. The aspartic acid at codon 364 is replaced by asparagine, an amino acid with highly similar properties. This alteration has been reported in muscular dystrophy cohorts (Park HJ et al. Clin Genet, 2017 03;91:403-410; Fan Y et al. J Med Genet, 2021 05;58:326-333). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
