Nucleotide substitutions within the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant is not likely to affect RNA splicing, but this prediction has not been confirmed by published transcriptional studies. This variant has not been reported in the literature in individuals with ATM-related disease. ClinVar contains an entry for this variant (Variation ID: 246116). This variant is not present in population databases (ExAC no frequency). This sequence change falls in intron 40 of the ATM gene. It does not directly change the encoded amino acid sequence of the ATM protein, but it affects a nucleotide within the consensus splice site of the intron. -

This variant is denoted ATM c.6006+4_6006+5delAA or IVS40+4_IVS40+5delAA and consists of a deletion of two nucleotides at the +4 to +5 position in intron 40 of the ATM gene. The normal sequence with the bases that are deleted in braces is Ggta[aa]tatt, where the capital letters are exonic and lowercase are intronic. Multiple in silico models predict this variant to weaken or destroy the nearby natural donor site, and to possibly cause abnormal gene splicing. However, in the absence of RNA or functional studies, the actual effect of this variant is unknown. ATM c.6006+4_6006+5delAA was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. This variant has not, to our knowledge, been published in the literature as pathogenic or benign. The nucleotides that are deleted are not conserved across species. Based on the currently available information, we consider ATM c.6006+4_6006+5delAA to be a variant of uncertain significance. -
