Homozygous familial hypercholesterolemia Pathogenic:1
The p.Arg737LysfsX45 variant in LDLR has been reported in one individual with familial hypercholesterolemia (FH; Mozas 2000) and has also been identified in 0.003% (1/30616) of South Asian chromosomes by gnomAD (http://gnomad.broadinstitute.org). This frequency is low enough to be consistent with the frequency of FH in the general population. This variant is predicted to cause a frameshift, which alters the protein’s amino acid sequence beginning at position 737 and leads to a premature termination codon 45 amino acids downstream. This alteration is then predicted to lead to a truncated or absent protein. Loss of function of the LDLR gene is an established disease mechanism in autosomal dominant FH. In summary, although additional studies are required to fully establish its clinical significance, this variant meets criteria to be classified as likely pathogenic for autosomal dominant FH. The c.2207dupT pathogenic mutation, located in coding exon 15 of the LDLR gene, results from a duplication of T at nucleotide position 2207, causing a translational frameshift with a predicted alternate stop codon (p.R737Kfs*45). This alteration has been reported in individuals with familial hypercholesterolemia (FH) (Mozas P et al. Hum Mutat, 2000 May;15:483-4; Ambry internal data). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
