Genes affected
TCF12 (HGNC:11623): (transcription factor 12) The protein encoded by this gene is a member of the basic helix-loop-helix (bHLH) E-protein family that recognizes the consensus binding site (E-box) CANNTG. This encoded protein is expressed in many tissues, among them skeletal muscle, thymus, B- and T-cells, and may participate in regulating lineage-specific gene expression through the formation of heterodimers with other bHLH E-proteins. Several alternatively spliced transcript variants of this gene have been described, but the full-length nature of some of these variants has not been determined. [provided by RefSeq, Jul 2008]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 18 ACMG points.

PVS1
Loss of function variant, product undergoes nonsense mediated mRNA decay. LoF is a known mechanism of disease.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 15-57232728-C-G is Pathogenic according to our data. Variant chr15-57232728-C-G is described in ClinVar as [Pathogenic]. Clinvar id is 55911.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars. Variant chr15-57232728-C-G is described in Lovd as [Pathogenic]. Variant chr15-57232728-C-G is described in Lovd as [Likely_pathogenic].

De novo variant with confirmed parentage in a patient referred for genetic testing at GeneDx; however, the reported clinical features are only partially consistent with the features typically observed in individuals with pathogenic variants in this gene; Nonsense variant predicted to result in protein truncation or nonsense mediated decay in a gene for which loss-of-function is a known mechanism of disease; Not observed at significant frequency in large population cohorts (gnomAD); This variant is associated with the following publications: (PMID: 25271085, 23354436) -

This sequence change creates a premature translational stop signal (p.Ser281*) in the TCF12 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in TCF12 are known to be pathogenic (PMID: 23354436, 32620954). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with clinical features of TCF12-related conditions (PMID: 23354436, 25271085). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 55911). For these reasons, this variant has been classified as Pathogenic. -
