Genes affected
CHD7 (HGNC:20626): (chromodomain helicase DNA binding protein 7) This gene encodes a protein that contains several helicase family domains. Mutations in this gene have been found in some patients with the CHARGE syndrome. Two transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Oct 2015]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 18 ACMG points.

PVS1
Loss of function variant, product undergoes nonsense mediated mRNA decay. LoF is a known mechanism of disease.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 8-60742366-C-T is Pathogenic according to our data. Variant chr8-60742366-C-T is described in ClinVar as [Pathogenic]. Clinvar id is 279757.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Pathogenic, criteria provided, single submitterclinical testingGeneDxAug 24, 2015The R312X nonsense variant in the CHD7 gene has been reported previously in association with CHARGE syndrome and has been observed at GeneDx in several patients referred for CHD7 sequence analysis (Writzl et al., 2007; Bartels et al., 2010). Approximately 45% of CHD7 pathogenic variants are nonsense changes predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. Therefore, we interpret R312X to be a pathogenic variant. -
CHARGE association Pathogenic:2
Pathogenic, criteria provided, single submitterclinical testing3billionJan 03, 2022The variant has been reported at least twice as pathogenic/likely pathogenic with clinical assertions and evidence for the classification (ClinVar ID: VCV000279757, PMID:17684005). Stop-gained (nonsense): predicted to result in a loss or disruption of normal protein function through nonsense-mediated decay (NMD) or protein truncation. Multiple pathogenic variants are reported downstream of the variant (PVS1_VS). It is not observed in the gnomAD v2.1.1 dataset (PM2_M). Therefore, this variant is classified as pathogenic according to the recommendation of ACMG/AMP guideline. -
Pathogenic, criteria provided, single submitterclinical testingInvitaeSep 20, 2022This sequence change creates a premature translational stop signal (p.Arg312*) in the CHD7 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in CHD7 are known to be pathogenic (PMID: 22461308, 25077900). For these reasons, this variant has been classified as Pathogenic. ClinVar contains an entry for this variant (Variation ID: 279757). This premature translational stop signal has been observed in individuals with clinical features of CHARGE syndrome (PMID: 17684005, 24368733, 31395954). This variant is not present in population databases (gnomAD no frequency). -
