Genes affected
PKHD1 (HGNC:9016): (PKHD1 ciliary IPT domain containing fibrocystin/polyductin) The protein encoded by this gene is predicted to have a single transmembrane (TM)-spanning domain and multiple copies of an immunoglobulin-like plexin-transcription-factor domain. Alternative splicing results in two transcript variants encoding different isoforms. Other alternatively spliced transcripts have been described, but the full length sequences have not been determined. Several of these transcripts are predicted to encode truncated products which lack the TM and may be secreted. Mutations in this gene cause autosomal recessive polycystic kidney disease, also known as polycystic kidney and hepatic disease-1. [provided by RefSeq, Jul 2008]

Uncertain significance, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpJul 09, 2019This sequence change replaces arginine with histidine at codon 880 of the PKHD1 protein (p.Arg880His). The arginine residue is moderately conserved and there is a small physicochemical difference between arginine and histidine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with PKHD1-related disease. ClinVar contains an entry for this variant (Variation ID: 282054). Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Uncertain significance, no assertion criteria providedclinical testingPreventionGenetics, part of Exact SciencesApr 12, 2024The PKHD1 c.2639G>A variant is predicted to result in the amino acid substitution p.Arg880His. To our knowledge, this variant has not been reported in the literature. This variant is reported in 0.012% of alleles in individuals of African descent in gnomAD. Of note, a different substitution at the same codon, defined as c.2639G>C (p.Arg880Pro), was reported with a pathogenic variant c.107C>T (p.Thr36Met) in an individual with polycystic kidney disease, but the clinical significance was uncertain (Supplementary Table S3 and S4 of Burgmaier et al. 2021. PubMed ID: 33940108). At this time, the clinical significance of the c.2639G>A (p.Arg880His) variant is uncertain due to the absence of conclusive functional and genetic evidence. -
