The encoded protein is involved in the ubiquitination and degradation of hypoxia-inducible-factor (HIF), which is a transcription factor that plays a central role in the regulation of gene expression by oxygen. In addition to oxygen-related gene expression, this protein plays a role in many other cellular processes including cilia formation, cytokine signaling, regulation of senescence, and formation of the extracellular matrix. This missense change has been observed in individual(s) with clinical features of von Hippel-Lindau syndrome (PMID: 19558618; Invitae). This variant is not present in population databases (gnomAD no frequency). This sequence change replaces proline, which is neutral and non-polar, with arginine, which is basic and polar, at codon 192 of the VHL protein (p.Pro192Arg). ClinVar contains an entry for this variant (Variation ID: 825946). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This variant disrupts the p.Pro192 amino acid residue in VHL. Other variant(s) that disrupt this residue have been observed in individuals with VHL-related conditions (PMID: 31397861; Invitae), which suggests that this may be a clinically significant amino acid residue. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt VHL protein function. -

The p.P192R variant (also known as c.575C>G), located in coding exon 3 of the VHL gene, results from a C to G substitution at nucleotide position 575. The proline at codon 192 is replaced by arginine, an amino acid with dissimilar properties. Another alteration at this same codon, p.P192L, has been described in multiple patients affected with pheochromocytomas (Ambry internal data; Neumann HPH et al. JAMA Netw Open, 2019 08;2:e198898). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
