Variant summary: HEXB c.448A>C (p.Thr150Pro) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. Consensus agreement among computation tools predict no significant impact on normal splicing. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 4e-06 in 251488 control chromosomes. c.448A>C has been reported in the literature in compound heterozygous individuals affected with Sandhoff Disease (Zampieri_2012, Gaignard_2013, Maegawa_2007). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 23046579, 17237499, 22848519). ClinVar contains an entry for this variant (Variation ID: 554994). Based on the evidence outlined above, the variant was classified as likely pathogenic. -

This sequence change replaces threonine, which is neutral and polar, with proline, which is neutral and non-polar, at codon 150 of the HEXB protein (p.Thr150Pro). This variant is present in population databases (no rsID available, gnomAD no frequency). This missense change has been observed in individual(s) with Sandhoff disease (PMID: 22848519, 23046579). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 554994). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
