Genes affected
HNRNPR (HGNC:5047): (heterogeneous nuclear ribonucleoprotein R) This gene encodes an RNA-binding protein that is a member of the spliceosome C complex, which functions in pre-mRNA processing and transport. The encoded protein also promotes transcription at the c-fos gene. Alternative splicing results in multiple transcript variants. There are pseudogenes for this gene on chromosomes 4, 11, and 10. ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. There are 9 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 1-23310755-G-GCC is Pathogenic according to our data. Variant chr1-23310755-G-GCC is described in ClinVar as [Pathogenic]. Clinvar id is 2609054.Status of the report is criteria_provided_single_submitter, 1 stars.

Pathogenic, criteria provided, single submitterclinical testingAmbry GeneticsAug 14, 2023The c.1608_1609dupGG (p.A537Gfs*37) alteration, located in exon 11 (coding exon 10) of the HNRNPR gene, consists of a duplication of GG at position 1608, causing a translational frameshift with a predicted alternate stop codon after 37 amino acids. This alteration occurs at the 3' terminus of the HNRNPR gene, is not expected to trigger nonsense-mediated mRNA decay, and only impacts the last 15.7% of the protein. However, premature stop codons are typically deleterious in nature, the impacted region is critical for protein function, and a significant portion of the protein is affected (Ambry internal data). This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). Based on the available evidence, this alteration is classified as pathogenic. -
