This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. This variant is classified as uncertain in ClinVar (Variation ID: 141579), and found in the general population with an overall allele frequency of 0.005% (13/276580 alleles) in the Genome Aggregation Database. The valine at codon 2540 is moderately conserved, but computational analyses (SIFT, PolyPhen-2) predict that this variant is tolerated. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4.8e-05 in 252044 control chromosomes. This frequency is not significantly higher than expected for a pathogenic variant in ATM causing Ataxia-Telangiectasia (4.8e-05 vs 0.004), allowing no conclusion about variant significance. c.7618G>A has been reported in the literature in individuals affected with breast cancer (e.g. Tung_2015, Tung_2016, Hauke_2018, Girard_2019), ovarian cancer (e.g. Stafford_2017), CLL (e.g. Tiao_2017), colorectal cancer (e.g. Pearlman_2016), and pancreatic cancer (e.g. Dudley_2018), but also in healthy controls (e.g. Renwick_2006, Tavtigian_2009). The variant has also been reported in an individual with dystonia (e.g. Pogoda_2019). These reports do not provide unequivocal conclusions about association of the variant with Ataxia-Telangiectasia. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Eight other clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. These laboratories cited the variant as uncertain significance (n=7) and likely benign (n=1). Based on the evidence outlined above, the variant was classified as uncertain significance. -
