The normal adult hemoglobin tetramer consists of two alpha chains and two beta chains. Mutant beta globin causes sickle cell anemia. Absence of beta chain causes beta-zero-thalassemia. Reduced amounts of detectable beta globin causes beta-plus-thalassemia. The order of the genes in the beta-globin cluster is 5'-epsilon -- gamma-G -- gamma-A -- delta -- beta--3'. Variant summary: HBB c.347C>A (p.Ala116Asp) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant was absent in 251276 control chromosomes. c.347C>A has been observed in the heterozygous state in multiple individuals affected with Beta Thalassemia and segregated with disease in at least two families consistent with autosomal dominant inheritance (Ohga_2003, Da_2021, Parikh_2005, Divoky_1993). These data indicate that the variant is very likely to be associated with disease. This variant is also known as Hb Hradec Kralove (or Hb HK). The following publications have been ascertained in the context of this evaluation (PMID: 33979976, 7693620, 14686490, 15486951). ClinVar contains an entry for this variant (Variation ID: 15526). Based on the evidence outlined above, the variant was classified as pathogenic. -
