Variant Summary: The FBN1 c.1916G>C (p.Cys639Ser) variant involves a conserved nucleotide and lies in a conserved region within EGF-like #6 domain of the FBN1 protein. Alteration of cysteine in this domain is likely to disrupt disulfide binding, thus affecting secondary or tertiary structure of the protein or possibly impairing fibrillin interactions. In support of the deleterious effect of this variant, 5/5 in silico analyses predict a disease-causing/damaging outcome and the variant is absent in control population from ExAC (0/121408 chromosomes). In addition, another variant at the same codon c.1916G>A (p.Cys639Trp) has been identified in at least 5 patients presenting with either isolated EL or classical MFS (Li, 2014; Howarth, 2007; Robinson, 2012). Lastly, the variant was identified in an internal sample referred for genetic testing due to personal and FH that is highly specific for disease associated with alteration of FBN1 gene. By applying ACMG rules (PM1, PM2, PM5, PP3, PP4), the variant has been classified as Likely Pathogenic. -
