The BARD1 p.Arg194Lys variant was identified in 2 of 7838 proband chromosomes (frequency: 0.0003) from individuals or families with breast or ovarian cancer (Tung 2016, Ramus 2015). The variant was also identified in dbSNP (ID: rs181748854) as "With Uncertain significance allele", ClinVar (classified as likely benign by Ambry Genetics; as uncertain significance by Invitae and GeneDx). The variant was identified in control databases in 6 of 270676 chromosomes at a frequency of 0.000022 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: Latino in 1 of 33390 chromosomes (freq: 0.00003), European in 5 of 124528 chromosomes (freq: 0.00004); it was not observed in the African, Other, Ashkenazi Jewish, East Asian, Finnish, and South Asian populations. The p.Arg194 residue is not conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; however, this information is not predictive enough to rule out pathogenicity. The variant occurs outside of the splicing consensus sequence and 1 of 4 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) predict a greater than 10% difference in splicing; this is not very predictive of pathogenicity. The BARD1 c.581G>A (p.Arg194Lys) variant has been reported in the published literature in individuals with breast cancer (PMID: 31371347 (2019), 26976419 (2016)) as well as unaffected individuals (PMID: 33471991 (2021) see also LOVD (https://databases.lovd.nl/shared/variants/BARD1), 26315354 (2015)). An experimental study reports the variant does not impact BARD1 homology directed repair (HDR) (PMID: 30925164 (2019)). The frequency of this variant in the general population, 0.000031 (4/127040 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -

This sequence change replaces arginine, which is basic and polar, with lysine, which is basic and polar, at codon 194 of the BARD1 protein (p.Arg194Lys). This variant is present in population databases (rs181748854, gnomAD 0.004%). This missense change has been observed in individual(s) with breast cancer (PMID: 26976419, 31371347). ClinVar contains an entry for this variant (Variation ID: 142942). An algorithm developed to predict the effect of missense changes on protein structure and function outputs the following: PolyPhen-2: "Benign". The lysine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. Experimental studies have shown that this missense change does not substantially affect BARD1 function (PMID: 30925164). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Five of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 2e-05 in 251682 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.581G>A has been reported in the literature in individuals affected with Breast Cancer without strong evidence for causality (Toh_2019, Tung_2016) and unaffected controls (Dorling_2021). These reports do not provide unequivocal conclusions about association of the variant with Breast Cancer. In BARD1 variant homology-directed repair (HDR) assays, the variant was found to be functional (Adamovich_2019). Five clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 and classified the variant as benign (n=1), likely benign (n=1) and VUS (n=3).
