SLC22A5 (HGNC:10969): (solute carrier family 22 member 5) Polyspecific organic cation transporters in the liver, kidney, intestine, and other organs are critical for elimination of many endogenous small organic cations as well as a wide array of drugs and environmental toxins. The encoded protein is a plasma integral membrane protein which functions both as an organic cation transporter and as a sodium-dependent high affinity carnitine transporter. The encoded protein is involved in the active cellular uptake of carnitine. Mutations in this gene are the cause of systemic primary carnitine deficiency (CDSP), an autosomal recessive disorder manifested early in life by hypoketotic hypoglycemia and acute metabolic decompensation, and later in life by skeletal myopathy or cardiomyopathy. The observed missense c.43G>Tp.Gly15Trp variant in SLC22A5 gene has been reported previously in homozygous and compound heterozygous states in multiple individuals affected with primary carnitine deficiency El-Hattab et al., 2010; Li et al., 2010; Rose et al., 2012. Experimental studies have shown that this missense change affects SLC22A5 function Rose et al., 2012. The p.Gly15Trp variant is present with allele frequency of 0.01% in gnomAD Exomes. This variant has been submitted to the ClinVar database as Likely Pathogenic/ Pathogenic multiple submissions. Multiple lines of computational evidence Polyphen - Probably Damaging, SIFT - Damaging and MutationTaster - Disease causing predict a damaging effect on protein structure and function for this variant. The reference amino acid of p.Gly15Trp in SLC22A5 is predicted as conserved by GERP++ and PhyloP across 100 vertebrates. The amino acid Gly at position 15 is changed to a Trp changing protein sequence and it might alter its composition and physico-chemical properties. For these reasons, this variant has been classified as Pathogenic. In absence of another reportable variant in SLC22A5 gene, the molecular diagnosis is not confirmed. -
