CFTR (HGNC:1884): (CF transmembrane conductance regulator) This gene encodes a member of the ATP-binding cassette (ABC) transporter superfamily. The encoded protein functions as a chloride channel, making it unique among members of this protein family, and controls ion and water secretion and absorption in epithelial tissues. Channel activation is mediated by cycles of regulatory domain phosphorylation, ATP-binding by the nucleotide-binding domains, and ATP hydrolysis. Mutations in this gene cause cystic fibrosis, the most common lethal genetic disorder in populations of Northern European descent. The most frequently occurring mutation in cystic fibrosis, DeltaF508, results in impaired folding and trafficking of the encoded protein. This sequence change replaces isoleucine with methionine at codon 506 of the CFTR protein (p.Ile506Met). The isoleucine residue is highly conserved and there is a small physicochemical difference between isoleucine and methionine. This variant is not present in population databases (ExAC no frequency). This missense change has been observed in individuals with cystic fibrosis (PMID: 7525963; Invitae). ClinVar contains an entry for this variant (Variation ID: 53278). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). This variant disrupts the p.Ile506 amino acid residue in CFTR. Other variant(s) that disrupt this residue have been observed in individuals with CFTR-related conditions (PMID: 7509683, 16436643), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

This CFTR missense variant has been identified in multiple individuals with features of cystic fibrosis. This variant (rs1800092) is rare (<0.1%) in a large population dataset (gnomADv3.1.2: 1/152016 total alleles; 0.0007%; no homozygotes) and has been reported in ClinVar (Variation ID: 53278). Two bioinformatic tools queried predict that this substitution would be damaging and the isoleucine residue at this position is evolutionarily conserved across all species assessed. We consider CFTR c.1518C>G to be likely pathogenic. -

Variant summary: CFTR c.1518C>G (p.Ile506Met) results in a conservative amino acid change located in the ABC transporter-like, ATP-binding domain (IPR003439) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251284 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1518C>G has been reported in the literature with p.F508del in an individual reportedly affected with Cystic Fibrosis (example, Chevalier-Porst_1994). However, the sick kids database states that a diagnosis of CF was not confirmed in this individual based on personal correspondence with one of the authors of this study. Therefore, this report does not provide unequivocal conclusions about association of the variant with Cystic Fibrosis. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. One laboratory classified the variant as pathogenic and one laboratory classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
