The p.Arg1086Ser variant in CACNA1S has been reported in three individuals with malignant hyperthermia susceptibility (Toppin 2010 PMID: 20431982, Miller 2018 PMID: 30236257). It has also been identified in 0.016% (5/30616) of South Asian chromosomes by gnomAD (http://gnomad.broadinstitute.org). This variant has also been reported in ClinVar (Variation ID 156288). Computational prediction tools and conservation analysis suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. Another variant involving this codon (p.Arg1086His) has been identified in individuals with malignant hyperthermia susceptibility and is classified as "drug response" by the Pharcogenomics Knowledge Base (PharmGKB) expert panel. In summary, although additional studies are required to fully establish its clinical significance, the p.Arg1086Ser variant is likely pathogenic for autosomal dominant malignant hyperthermia susceptibility.
