FBN1 (HGNC:3603): (fibrillin 1) This gene encodes a member of the fibrillin family of proteins. The encoded preproprotein is proteolytically processed to generate two proteins including the extracellular matrix component fibrillin-1 and the protein hormone asprosin. Fibrillin-1 is an extracellular matrix glycoprotein that serves as a structural component of calcium-binding microfibrils. These microfibrils provide force-bearing structural support in elastic and nonelastic connective tissue throughout the body. Asprosin, secreted by white adipose tissue, has been shown to regulate glucose homeostasis. Mutations in this gene are associated with Marfan syndrome and the related MASS phenotype, as well as ectopia lentis syndrome, Weill-Marchesani syndrome, Shprintzen-Goldberg syndrome and neonatal progeroid syndrome. Variant summary: The FBN1 c.2858delT (p.Ile953Thrfs) variant results in a premature termination codon, predicted to cause a truncated or absent FBN1 protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory (e.g., c.3973dupG [p.Glul1325fs). The variant lies within the TGFBP #3 functional domain (UMD) and one in silico tool predicts a damaging outcome for this variant. This variant is absent from the large control database ExAC (0/115756 control chromosomes). In addition, the variant has been cataloged in patient databases, such as UMD (UMD IDs 2349 and 2905), and was reported in the literature in a MFS patient fulfilling the revised Ghent criteria and classified as causative by the authors (Baetens_2011).
