NM_000166.6:c.242T>A
Variant summary
Our verdict is Pathogenic. The variant received 11 ACMG points: 11P and 0B. PM1PM2PM5PP2PP3_Strong
The NM_000166.6(GJB1):c.242T>A(p.Leu81His) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. 12/21 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. L81F) has been classified as Pathogenic.

Charcot-Marie-Tooth Neuropathy X Uncertain:1
This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with GJB1-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This sequence change replaces leucine with histidine at codon 81 of the GJB1 protein (p.Leu81His). The leucine residue is highly conserved and there is a moderate physicochemical difference between leucine and histidine.
