The L601V variant has been reported in a female diagnosed with HCM at 7 years of age and parental testing confirmed this variant occurred de novo in this individual (Havndrup et al., 2003). The L601V variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Although the L601V variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties, this substitution occurs at a position that is conserved across species. Furthermore, in silico analysis predicts this variant is probably damaging to the protein structure/function. Lastly, a missense variant affecting the same residue (L601F) and missense variants in nearby residues (I519T, N602S, V606L, V606M, G607D) have been reported in the Human Gene Mutation Database in association with cardiomyopathy (Stenson et al., 2014), supporting the functional importance of this residue and region of the protein.

The variant is not observed in the gnomAD v4.1.0 dataset. Predicted Consequence/Location: The variant is located in a mutational hot spot and/or well-established functional domain in which established pathogenic variants have been reported (PMID: 29300372). In silico tool predictions suggest damaging effect of the variant on gene or gene product [REVEL: 0.78 (>=0.6, sensitivity 0.68 and specificity 0.92); 3Cnet: 0.99 (>=0.6, sensitivity 0.72 and precision 0.9)]. The same nucleotide change resulting in the same amino acid change has been previously reported to be associated with MYH7 related disorder (ClinVar ID: VCV000429937 /PMID: 12566107).A different missense change at the same codon (p.Leu601Phe) has been reported to be associated with MYH7 related disorder (ClinVar ID: VCV000574861 /PMID: 16858239). Therefore, this variant is classified as Likely pathogenic according to the recommendation of ACMG/AMP guideline.
