This sequence change replaces proline with threonine at codon 144 of the MYL2 protein (p.Pro144Thr). The proline residue is highly conserved and there is a small physicochemical difference between proline and threonine. In summary, this variant is a rare missense change with uncertain impact on protein function. There is no indication that it causes disease, but the available evidence is currently insufficient to prove that conclusively. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). However, a computational algorithm designed to assess the pathogenicity of variants in MYL2 with regard to hypertrophic cardiomyopathy predicted this sequence change to be deleterious. The algorithm has a sensitivity of 94% and a specificity of 89% (PMID: 21310275). This variant is present in population databases (rs777689913, ExAC 0.002%) but has not been reported in the literature in individuals with a MYL2-related disease. -
