The Hb Olympia variant (HBB: c.61G>A; p.Val21Met, also known as Val20Met when numbered from the mature protein, rs35890959) is reported in the literature in multiple individuals affected with erythrocytosis (Bento 2013, Gonzalez Fernandez 2009, Percy 2009, Stamatoyannopoulos 1973, Wajcman 2005, HbVar database and references therein). This variant has been observed to segregate with erythrocytosis in a family in a pattern consistent with autosomal dominant inheritance (Stamatoyannopoulos 1973). Further, while heterozygous individuals with this variant are reported with mild erythrocytosis, those carrying both Hb Olympia and a beta-thalassemia allele are reported to exhibit more severe symptoms (Wajcman 2005). The Hb Olympia variant is found on a single chromosome in the Genome Aggregation Database (1/251260 alleles), indicating it is not a common polymorphism. The valine at codon 21 is highly conserved, and functional studies indicate increased oxygen affinity of the variant protein (Stamatoyannopoulos 1973). Based on available information, this variant is considered to be pathogenic. Variant summary: HBB c.61G>A (p.Val21Met) also known as Hb Olympia results in a conservative amino acid change located in the Globin (IPR000971) of the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251260 control chromosomes. c.61G>A has been reported in the literature in multiple individuals affected with Erythrocytosis consistent with autosomal dominant inheritance (e.g. Filser_2022, Gonzalez Fernandez_2009, Percy_2009, Stamatoyannopoulos_1973) and shown to segregate with disease. These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 35052472, 18818920, 19734427, 4683875). ClinVar contains an entry for this variant (Variation ID: 15300).
