The TTN c.19063G>T, p.(Asp6355Tyr) variant (rs188878341; ClinVar Variation ID: 178247) is rare in the general population (<1% allele frequency in the Genome Aggregation Database) and has not been reported in the medical literature in association with dilated cardiomyopathy (DCM) or other TTN-related disease. The clinical relevance of rare missense variants in this gene, which are identified on average once per individual sequenced in affected populations (Herman 2012), is not well understood. Yet, evidence suggests that the vast majority of such missense variants do not contribute to the clinical outcome of DCM (Begay 2015). Thus, the clinical significance of the p.(Asp6355Tyr) variant cannot be determined with certainty. -

The Asp5111Tyr vari ant in TTN has not been previously reported in individuals with cardiomyopathy, but has been identified in 0.2% (9/3802) of African American chromosomes by the NHLBI Exome Sequencing Project (http://evs.gs.washington.edu/EVS/; dbSNP rs18887 8341). Computational prediction tools and conservation analysis do not provide s trong support for or against an impact to the protein. In summary, while the cli nical significance of the Asp5111Tyr variant is uncertain, its frequency suggest s that it is more likely to be benign. -

Variant summary: TTN c.15331G>T (p.Asp5111Tyr) results in a non-conservative amino acid change located in the I-band region of the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 0.00019 in 247214 control chromosomes, predominantly at a frequency of 0.0026 within the African or African-American subpopulation in the gnomAD database. The observed variant frequency within African or African-American control individuals in the gnomAD database is approximately 7-fold of the estimated maximal expected allele frequency for a pathogenic variant in TTN causing Dilated Cardiomyopathy phenotype (0.00039). c.15331G>T has been observed with other co-occurring variants in a case of sudden infant death (Campuzano_2018). This report does not provide unequivocal conclusions about association of the variant with Dilated Cardiomyopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported.
