Variant summary
Our verdict is Uncertain significance. The variant received 4 ACMG points: 4P and 0B. PM1PP3PP5
The NM_001277115.2(DNAH11):c.6527C>T(p.Ala2176Val) variant causes a missense change. The variant allele was found at a frequency of 0.00000744 in 1,613,482 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars).

Primary ciliary dyskinesia 7 Pathogenic:1Uncertain:1

The p.Ala2176Val variant in DNAH11 has not been previously reported in the literature in individuals with primary ciliary dyskinesia, but has been identified in 0.002% (1/44880) of East Asian chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP rs770279400). Although this variant has been seen in the general population in a heterozygous state, its frequency is not high enough to rule out a pathogenic role. This variant has also been reported in ClinVar (Variation ID: 410873) and has been interpreted as a variant of uncertain significance by Women's Health and Genetics/Laboratory Corporation of America (LabCorp) and as pathogenic by Labcorp Genetics (formerly Invitae). Computational prediction tools and conservation analyses suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. In summary, the clinical significance of the p.Ala2176Val variant is uncertain. ACMG/AMP Criteria applied: PP3, PM2_supporting (Richards 2015). -
Primary ciliary dyskinesia Pathogenic:1
This sequence change replaces alanine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 2176 of the DNAH11 protein (p.Ala2176Val). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with primary ciliary (Invitae). ClinVar contains an entry for this variant (Variation ID: 410873). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on DNAH11 protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic. -
not specified Uncertain:1
Variant summary: DNAH11 c.6527C>T (p.Ala2176Val) results in a non-conservative amino acid change located in the Dynein heavy chain, hydrolytic ATP-binding dynein motor region (IPR035699) of the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 4e-06 in 249030 control chromosomes. c.6527C>T has been observed in individual(s) affected with Primary Ciliary Dyskinesia (internal data). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. ClinVar contains an entry for this variant (Variation ID: 410873). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. -
