The p.G217* pathogenic mutation (also known as c.649G>T), located in coding exon 7 of the RAD51D gene, results from a G to T substitution at nucleotide position 649. This changes the amino acid from a glycine to a stop codon within coding exon 7. In a case-control study involving 3429 epithelial ovarian cancer patients, this mutation was reported in cis with RAD51D p.Q219* (c.655C>T) in one patient with high-grade serous epithelial ovarian cancer at age 73 (Song H et al. J. Clin. Oncol. 2015 Sep;33:2901-7). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -

Variant summary: RAD51D c.649G>T (p.Gly217X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory and associated with phenotype in HGMD. The variant allele was found at a frequency of 8e-06 in 251434 control chromosomes. c.649G>T has been reported in the literature in at least one individual affected with High Grade Serous Epithelial Ovarian Cancer (example Song_2015). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 and classified the variant as pathogenic (n=2) and likely pathogenic (n=1). Based on the evidence outlined above, the variant was classified as pathogenic. -
