Variant summary

Our verdict is Pathogenic. The variant received 15 ACMG points: 15P and 0B. PS3PM1PM4_SupportingPP5_Very_Strong

The NM_003060.4(SLC22A5):c.1181_1183delTGC(p.Leu394del) variant causes a disruptive inframe deletion change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.000031 in 1,614,218 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Likely pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV002103683: The most pronounced variant effect results in <10% of normal activity (Frigeni_2017)." and additional evidence is available in ClinVar. Synonymous variant affecting the same amino acid position (i.e. L394L) has been classified as Likely benign.

Genes affected
SLC22A5 (HGNC:10969): (solute carrier family 22 member 5) Polyspecific organic cation transporters in the liver, kidney, intestine, and other organs are critical for elimination of many endogenous small organic cations as well as a wide array of drugs and environmental toxins. The encoded protein is a plasma integral membrane protein which functions both as an organic cation transporter and as a sodium-dependent high affinity carnitine transporter. The encoded protein is involved in the active cellular uptake of carnitine. Mutations in this gene are the cause of systemic primary carnitine deficiency (CDSP), an autosomal recessive disorder manifested early in life by hypoketotic hypoglycemia and acute metabolic decompensation, and later in life by skeletal myopathy or cardiomyopathy. Alternative splicing of this gene results in multiple transcript variants. [provided by RefSeq, Apr 2015]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 15 ACMG points.

PS3
PS3 evidence extracted from ClinVar submissions: SCV002103683: The most pronounced variant effect results in <10% of normal activity (Frigeni_2017).; SCV000239182: Published functional studies in CHO cells demonstrate a damaging effect on protein function (PMID: 28841266)
PM1
In a hotspot region, there are 4 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 0 benign, 5 uncertain in NM_003060.4
PM4
Nonframeshift variant in NON repetitive region in NM_003060.4. Strenght limited to Supporting due to length of the change: 1aa.
PP5
Variant 5-132390809-GGCT-G is Pathogenic according to our data. Variant chr5-132390809-GGCT-G is described in ClinVar as Pathogenic/Likely_pathogenic. ClinVar VariationId is 25410.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
