The encoded preproprotein is proteolytically processed to generate multiple protein products, which may exhibit unique activities and specificities. These proteins may play a role in skin and hair morphogenesis, as well as anti-inflammatory and antimicrobial protection of mucous epithelia. Mutations in this gene may result in Netherton syndrome, a disorder characterized by ichthyosis, defective cornification, and atopy. This gene is present in a gene cluster on chromosome 5. This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 636 of the SPINK5 protein (p.Arg636Gln). This variant is present in population databases (rs758747211, gnomAD 0.006%). This variant has not been reported in the literature in individuals affected with SPINK5-related conditions. ClinVar contains an entry for this variant (Variation ID: 529157). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt SPINK5 protein function with a negative predictive value of 80%. The c.1907G>A (p.R636Q) alteration is located in exon 21 (coding exon 21) of the SPINK5 gene. This alteration results from a G to A substitution at nucleotide position 1907, causing the arginine (R) at amino acid position 636 to be replaced by a glutamine (Q).
