The same nucleotide change resulting in the same amino acid change has been previously reported as pathogenic/likely pathogenic with strong evidence (ClinVar ID: VCV000004337 /PMID: 11704758). A different missense change at the same codon (p.Val316Phe) has been reported to be associated with EIF2B2-related disorder (ClinVar ID: VCV000807410). Therefore, this variant is classified as Pathogenic (PS1_S, PM2_M, PM3_M, PM5_P, PP3_P) according to the recommendation of ACMG/AMP guideline.

This sequence change replaces valine, which is neutral and non-polar, with aspartic acid, which is acidic and polar, at codon 316 of the EIF2B2 protein (p.Val316Asp). This variant is present in population databases (rs104894426, gnomAD 0.006%). This missense change has been observed in individuals with leukoencephalopathy with vanishing white matter (PMID: 11704758, 33432707). ClinVar contains an entry for this variant (Variation ID: 4337). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt EIF2B2 protein function with a positive predictive value of 80%. Experimental studies have shown that this missense change affects EIF2B2 function (PMID: 14993275, 15060152). For these reasons, this variant has been classified as Pathogenic.

Variant summary: EIF2B2 c.947T>A (p.Val316Asp) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 251478 control chromosomes. c.947T>A has been reported in the literature in individuals affected with Leukoencephalopathy With Vanishing White Matter, in compound heterozygosity with pathogenic variants (Leegwater_2001, Slynko_2020). These data indicate that the variant is likely to be associated with disease. Experimental studies reported the vairant to have significantly reduced eIF2B activity (Richardson_2004) and partial loss of activity with reduced efficienciency to form holocomplexes (Li_2004). One clinical diagnostic laboratory has submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. One laboratory classified the variant as likely pathogenic. Based on the evidence outlined above, the variant was classified as likely pathogenic.
