The Cys192Ser variant in SDHB has not been previously reported in any other fami lies with hereditary paragangliomas and pheochromocytomas or GIST. This variant has not been identified in large population studies. Computational analyses (bio chemical amino acid properties, conservation, AlignGVGD, PolyPhen2, and SIFT) su ggest that this variant may impact the protein, though this information is not p redictive enough to determine pathogenicity. Finally, similar variants at the sa me position (Cys192Arg, Cys192Tyr) have been reported in individuals with heredi tary paragangliomas and pheochromocytomas (Neuman 2002, Benn 2006, Lefebre 2012) . While collectively, this information supports that the Cys192Ser variant is li kely pathogenic, additional studies are needed to fully establish its clinical s ignificance. -

This sequence change replaces cysteine, which is neutral and slightly polar, with serine, which is neutral and polar, at codon 192 of the SDHB protein (p.Cys192Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with paraganglioma (internal data). ClinVar contains an entry for this variant (Variation ID: 44645). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt SDHB protein function with a positive predictive value of 80%. This variant disrupts the p.Cys192 amino acid residue in SDHB. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 12000816, 16317055, 17200167, 18382370, 19454582, 22517554, 22835832, 25371406, 27785149, 28374168, 34906457). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing.
