Genes affected
LGI1 (HGNC:6572): (leucine rich glioma inactivated 1) This gene encodes a member of the secreted leucine-rich repeat (LRR) superfamily and shares homology with members of the SLIT protein family. The encoded protein may regulate the activity of voltage-gated potassium channels and may be involved in neuronal growth regulation and cell survival. This gene is rearranged as a result of translocations in glioblastoma cell lines, and it is frequently down-regulated or rearranged in malignant gliomas. Mutations in this gene result in autosomal dominant lateral temporal epilepsy. Alternative splicing results in multiple transcript variants. [provided by RefSeq, Apr 2015]

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. There are 9 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 10-93797385-T-G is Pathogenic according to our data. Variant chr10-93797385-T-G is described in ClinVar as [Pathogenic]. Clinvar id is 464744.Status of the report is criteria_provided_single_submitter, 1 stars.

Autosomal dominant epilepsy with auditory features Pathogenic:1
Pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpMar 05, 2021For these reasons, this variant has been classified as Pathogenic. A different nonsense variant that is further downstream (p.Arg474*) has been shown to segregate with disease and determined to be pathogenic (PMID: 11978770, 15349881). This suggests that a domain critical for LGI1 protein function is likely disrupted in these variants. This variant has not been reported in the literature in individuals with LGI1-related disease. This variant is not present in population databases (ExAC no frequency). This sequence change results in a premature translational stop signal in the LGI1 gene (p.Leu419*). While this is not anticipated to result in nonsense mediated decay, it is expected to delete the last 139 amino acids of the LGI1 protein. -
