GNPTAB (HGNC:29670): (N-acetylglucosamine-1-phosphate transferase subunits alpha and beta) This gene encodes two of three subunit types of the membrane-bound enzyme N-acetylglucosamine-1-phosphotransferase, a heterohexameric complex composed of two alpha, two beta, and two gamma subunits. The encoded protein is proteolytically cleaved at the Lys928-Asp929 bond to yield mature alpha and beta polypeptides while the gamma subunits are the product of a distinct gene (GeneID 84572). In the Golgi apparatus, the heterohexameric complex catalyzes the first step in the synthesis of mannose 6-phosphate recognition markers on certain oligosaccharides of newly synthesized lysosomal enzymes. These recognition markers are essential for appropriate trafficking of lysosomal enzymes. Mutations in this gene have been associated with both mucolipidosis II and mucolipidosis IIIA.[provided by RefSeq, May 2010]

Studies have shown that disruption of this splice site results in skipping of exon 13 and introduces a premature termination codon (PMID: 16116615). The c.2715+1G>A variant has been reported in at least five studies, in which it is found in a compound heterozygous state with a second variant in 19 individuals, including in three with mucolipidosis type II and in 16 with mucolipidosis type III alpha/beta. In patients, serum levels of lysosomal enzymes were significantly increased compared to controls (Paik et al. 2005; Otomo et al. 2009; Liu et al. 2016; Ho et al. 2018; Wang et al. 2019). In one study, the c.2715+1G>A variant was found in 28% of affected Chinese individuals (Liu et al. 2016). Control data are unavailable for this variant, which is reported at a frequency of 0.000163 in the East Asian population of the Genome Aggregation Database. RT-PCR confirmed that the c.2715+1G>A variant results in a deletion of 1103 bp of exon 13 resulting in skipping of this exon (Paik et al. 2005). Based on the collective evidence, the c.2715+1G>A variant is classified as pathogenic for GNPTAB-related mucolipidosis. - |
