Genes affected
DICER1 (HGNC:17098): (dicer 1, ribonuclease III) This gene encodes a protein possessing an RNA helicase motif containing a DEXH box in its amino terminus and an RNA motif in the carboxy terminus. The encoded protein functions as a ribonuclease and is required by the RNA interference and small temporal RNA (stRNA) pathways to produce the active small RNA component that represses gene expression. This protein also acts as a strong antiviral agent with activity against RNA viruses, including the Zika and SARS-CoV-2 viruses. Alternative splicing results in multiple transcript variants. This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score and internal cut-off values, a variant classified as benign is not then subjected to further curation. The score for this variant resulted in a classification of benign for this disease. -

The DICER1 c.773G>T variant is predicted to result in the amino acid substitution p.Gly258Val. To our knowledge, this variant has not been reported in individuals with DICER1-associated disorders in the literature. This variant is reported in 0.015% of alleles in individuals of European (Non-Finnish) descent in gnomAD and has conflicting classifications of pathogenicity from benign to uncertain significance in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/315113/). Although we suspect that this variant may be benign, at this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -

Significance:Uncertain significance
Review Status:criteria provided, single submitter
Collection Method:clinical testing

The c.773G>T (p.G258V) alteration is located in exon 7 (coding exon 6) of the DICER1 gene. This alteration results from a G to T substitution at nucleotide position 773, causing the glycine (G) at amino acid position 258 to be replaced by a valine (V). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
