This gene encodes a muscle-specific member of the calpain large subunit family that specifically binds to titin. Mutations in this gene are associated with limb-girdle muscular dystrophies type 2A. This sequence change replaces arginine, which is basic and polar, with leucine, which is neutral and non-polar, at codon 197 of the CAPN3 protein (p.Arg197Leu). This variant is present in population databases (rs768426565, gnomAD 0.03%). This missense change has been observed in individual(s) with autosomal recessive limb-girdle muscular dystrophy (PMID: 15689361, 16141003, 30564623, 33337384). ClinVar contains an entry for this variant (Variation ID: 446982). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt CAPN3 protein function with a negative predictive value of 80%. This variant disrupts the p.Arg197 amino acid residue in CAPN3. Other variant(s) that disrupt this residue have been observed in individuals with CAPN3-related conditions (PMID: 16141003, 33337384), which suggests that this may be a clinically significant amino acid residue.
