Genes affected
FBN1 (HGNC:3603): (fibrillin 1) This gene encodes a member of the fibrillin family of proteins. The encoded preproprotein is proteolytically processed to generate two proteins including the extracellular matrix component fibrillin-1 and the protein hormone asprosin. Fibrillin-1 is an extracellular matrix glycoprotein that serves as a structural component of calcium-binding microfibrils. These microfibrils provide force-bearing structural support in elastic and nonelastic connective tissue throughout the body. Asprosin, secreted by white adipose tissue, has been shown to regulate glucose homeostasis. Mutations in this gene are associated with Marfan syndrome and the related MASS phenotype, as well as ectopia lentis syndrome, Weill-Marchesani syndrome, Shprintzen-Goldberg syndrome and neonatal progeroid syndrome. The p.C832F pathogenic mutation (also known as c.2495G>T), located in coding exon 20 of the FBN1 gene, results from a G to T substitution at nucleotide position 2495. The cysteine at codon 832 is replaced by phenylalanine, an amino acid with highly dissimilar properties, and is located in the cbEGF-like domain #09. The majority of FBN1 mutations identified to date have involved the substitution or generation of cysteine residues within cbEGF domains (Vollbrandt T et al. J Biol Chem. 2004;279(31):32924-32931). This variant has been reported in individuals with a clinical diagnosis of Marfan syndrome (Ambry internal data; Hung CC et al. Ann. Hum. Genet., 2009 Nov;73:559-67). Based on internal structural assessment, this alteration eliminates a structurally critical disulfide in the the structurally sensitive cb EGF-like domain #09. Furthermore, additional amino acid substitutions at this codon, including p.C832Y, p.C832S, p.C832R, have also been reported in individuals with Marfan syndrome (Liu WO et al. Genet. Test., 1997-1998;1:237-42; Groth KA et al. Genet. Med., 2017 07;19:772-777; Budisteanu M et al. Clin. Dysmorphol., 2017 Jul;26:187-189). Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation.

Affects a cysteine residue within a calcium-binding EGF-like domain of the FBN1 gene, which may affect disulfide bonding and is predicted to alter the structure and function of the protein; cysteine substitutions in the calcium-binding EGF-like domains represent the majority of pathogenic missense changes associated with FBN1-related disorders (PMID: 12938084); Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; This variant is associated with the following publications: (PMID: 12938084, 19839986, 10464652, 10486319, 16222657, 17657824, 24199744, 29357934, 31098894)

This variant is not present in population databases (gnomAD no frequency). For these reasons, this variant has been classified as Pathogenic. This variant disrupts the p.Cys832 amino acid residue in FBN1. Other variant(s) that disrupt this residue have been observed in individuals with FBN1-related conditions (PMID: 27906200, 28277377), which suggests that this may be a clinically significant amino acid residue. This variant affects a cysteine residue in the EGF-like, TGFBP or hybrid motif domains of FBN1. Cysteine residues are believed to be involved in intramolecular disulfide bridges and have been shown to be important for FBN1 protein structure (PMID: 16905551, 19349279). In addition, missense substitutions affecting cysteine residues within these domains are significantly overrepresented among patients with Marfan syndrome (PMID: 16571647, 17701892). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt FBN1 protein function. ClinVar contains an entry for this variant (Variation ID: 963640). This missense change has been observed in individuals with clinical features of Marfan syndrome (PMID: 19839986; Invitae). This sequence change replaces cysteine, which is neutral and slightly polar, with phenylalanine, which is neutral and non-polar, at codon 832 of the FBN1 protein (p.Cys832Phe).
