This sequence change replaces arginine, which is basic and polar, with proline, which is neutral and non-polar, at codon 165 of the FBN1 protein (p.Arg165Pro). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of Marfan syndrome and/or Marfan syndrome (PMID: 31279624; Invitae). ClinVar contains an entry for this variant (Variation ID: 406352). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt FBN1 protein function with a positive predictive value of 95%. This variant disrupts the p.Arg165 amino acid residue in FBN1. Other variant(s) that disrupt this residue have been determined to be pathogenic (Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -

The R165P variant of uncertain significance in the FBN1 gene has not been published as pathogenic or been reported as benign to our knowledge. This variant was not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server), indicating it is not a common benign variant. The R165P variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. Additionally, this substitution occurs at a position that is highly conserved across species and in silico analysis predicts this variant is probably damaging to the protein structure/function. Finally, although the R165P variant is within an EGF-like domain of the FBN1 gene, it does not affect a Cysteine residue, as the majority of pathogenic missense changes associated with Marfan syndrome do (Collod-Beroud et al., 2003).Therefore, based on the currently available information, it is unclear whether this variant is pathogenic or rare benign. -
