The c.1823_1826delAGAA pathogenic mutation, located in coding exon 9 of the BRCA1 gene, results from a deletion of 4 nucleotides at nucleotide positions 1823 to 1826, causing a translational frameshift with a predicted alternate stop codon (p.K608Ifs*3). This alteration has been reported in multiple individuals diagnosed with breast and/or ovarian cancer (Gayther SA et al. Am. J. Hum. Genet. 1999 Oct;65:1021-9; Shattuck-Eidens D et al. JAMA. 1997 Oct;278:1242-50; Thomassen M et al. Acta Oncol. 2008;47:772-7; Song H et al. Hum. Mol. Genet. 2014 Sep;23:4703-9; Loizidou MA et al. Clin. Genet. 2017 Apr;91:611-615; Heramb C et al. Hered Cancer Clin Pract, 2018 Jan;16:3; Bertelsen B et al. NPJ Genom Med, 2019 Jun;4:13). This alteration was also identified in a large, worldwide study of BRCA1/2 mutation positive families (Rebbeck TR et al. Hum Mutat, 2018 05;39:593-620). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -

This premature translational stop signal has been observed in individual(s) with breast and ovarian cancer (PMID: 9333265, 10486320, 24504028, 24728189, 25452441). This variant is also known as 1942del4 and 608_609del. ClinVar contains an entry for this variant (Variation ID: 54361). For these reasons, this variant has been classified as Pathogenic. -
Variant summary: BRCA1 c.1823_1826delAGAA (p.Lys608IlefsX3) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant allele was found at a frequency of 3.9e-06 in 254132 control chromosomes. c.1823_1826delAGAA has been reported in the literature in multiple individuals affected with Hereditary Breast And Ovarian Cancer Syndrome (examples- Shattuck-Eidens_1997, Gayther_1999, Judkins_2005, Cunningham_2014, Couch_2015). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Eight other ClinVar submitters, including two expert panels, (evaluation after 2014) have cited the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
