BRCA1 (HGNC:1100): (BRCA1 DNA repair associated) This gene encodes a 190 kD nuclear phosphoprotein that plays a role in maintaining genomic stability, and it also acts as a tumor suppressor. The BRCA1 gene contains 22 exons spanning about 110 kb of DNA. The encoded protein combines with other tumor suppressors, DNA damage sensors, and signal transducers to form a large multi-subunit protein complex known as the BRCA1-associated genome surveillance complex (BASC). This gene product associates with RNA polymerase II, and through the C-terminal domain, also interacts with histone deacetylase complexes. This protein thus plays a role in transcription, DNA repair of double-stranded breaks, and recombination. Mutations in this gene are responsible for approximately 40% of inherited breast cancers and more than 80% of inherited breast and ovarian cancers. Alternative splicing plays a role in modulating the subcellular localization and physiological function of this gene. Many alternatively spliced transcript variants, some of which are disease-associated mutations, have been described for this gene, but the full-length natures of only some of these variants has been described. The c.1823_1826delAGAA pathogenic mutation, located in coding exon 9 of the BRCA1 gene, results from a deletion of 4 nucleotides at nucleotide positions 1823 to 1826, causing a translational frameshift with a predicted alternate stop codon (p.K608Ifs*3). This alteration has been reported in multiple individuals diagnosed with breast and/or ovarian cancer (Gayther SA et al. Am. J. Hum. Genet. 1999 Oct;65:1021-9; Shattuck-Eidens D et al. JAMA. 1997 Oct;278:1242-50; Thomassen M et al. Acta Oncol. 2008;47:772-7; Song H et al. Hum. Mol. Genet. 2014 Sep;23:4703-9; Loizidou MA et al. Clin. Genet. 2017 Apr;91:611-615; Heramb C et al. Hered Cancer Clin Pract, 2018 Jan;16:3; Bertelsen B et al. NPJ Genom Med, 2019 Jun;4:13). Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant allele was found at a frequency of 3.9e-06 in 254132 control chromosomes. c.1823_1826delAGAA has been reported in the literature in multiple individuals affected with Hereditary Breast And Ovarian Cancer Syndrome (examples- Shattuck-Eidens_1997, Gayther_1999, Judkins_2005, Cunningham_2014, Couch_2015). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Eight other ClinVar submitters, including two expert panels, (evaluation after 2014) have cited the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
