Genes affected
RYR1 (HGNC:10483): (ryanodine receptor 1) This gene encodes a ryanodine receptor found in skeletal muscle. The encoded protein functions as a calcium release channel in the sarcoplasmic reticulum but also serves to connect the sarcoplasmic reticulum and transverse tubule. Mutations in this gene are associated with malignant hyperthermia susceptibility, central core disease, and minicore myopathy with external ophthalmoplegia. Alternatively spliced transcripts encoding different isoforms have been described. [provided by RefSeq, Jul 2008]

This sequence change replaces leucine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 2286 of the RYR1 protein (p.Leu2286Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with myopathy, including autosomal recessive congenital myopathy (PMID: 32403337, 32528171; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 1980358). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on RYR1 protein function. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

This variant was identified in the heterozygous state in seven families. The most prevalent features in these patients were exertional myalgia, high CK levels, cramps and muscle hypertrophy. Further details are described in Segarra-Casas et al, 2025 (DOI: 10.1111/ene.16471). Criteria applied: PM1, PM, PM3, PP1_supporting, PP3 (Richards et al, 2015) -
