The encoded protein functions as a calcium release channel in the sarcoplasmic reticulum but also serves to connect the sarcoplasmic reticulum and transverse tubule. Mutations in this gene are associated with malignant hyperthermia susceptibility, central core disease, and minicore myopathy with external ophthalmoplegia. Likely pathogenic, criteria provided, single submitterclinical testingInvitaeFeb 07, 2019This sequence change replaces serine with tyrosine at codon 4830 of the RYR1 protein (p.Ser4830Tyr). The serine residue is highly conserved and there is a large physicochemical difference between serine and tyrosine. This sequence change is located in a region of the RYR1 protein where a significant number of previously reported RYR1 missense mutations are found (PMID: 16084090). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has been observed to be de novo in an individual affected with generalized muscle weakness (Invitae). This variant is not present in population databases (ExAC no frequency). -
