The variant allele was found at a frequency of 0.00000633 in 1,579,308 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Uncertain significance in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G196V) has been classified as Likely pathogenic.

Cardiovascular phenotype Uncertain:1
The p.G196R variant (also known as c.586G>C), located in coding exon 1 of the FKRP gene, results from a G to C substitution at nucleotide position 586. The glycine at codon 196 is replaced by arginine, an amino acid with dissimilar properties. This variant has been reported in muscular dystrophy cohorts; however, clinical details were limited in some cases (Kesari A et al. Am J Pathol, 2008 Nov;173:1476-87; McMillan HJ et al. Can J Neurol Sci, 2013 Nov;40:875-7; Gonzalez-Perez P et al. Neuromuscul Disord, 2020 Mar;30:213-218; Leung DG et al. BMC Neurol, 2020 May;20:196). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
Walker-Warburg congenital muscular dystrophy Uncertain:1
This sequence change replaces glycine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 196 of the FKRP protein (p.Gly196Arg). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individuals with limb-girdle muscular dystrophy (PMID: 18832576, 32429923). ClinVar contains an entry for this variant (Variation ID: 282824). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
