BARD1 (HGNC:952): (BRCA1 associated RING domain 1) This gene encodes a protein which interacts with the N-terminal region of BRCA1. In addition to its ability to bind BRCA1 in vivo and in vitro, it shares homology with the 2 most conserved regions of BRCA1: the N-terminal RING motif and the C-terminal BRCT domain. The RING motif is a cysteine-rich sequence found in a variety of proteins that regulate cell growth, including the products of tumor suppressor genes and dominant protooncogenes. This protein also contains 3 tandem ankyrin repeats. The BARD1/BRCA1 interaction is disrupted by tumorigenic amino acid substitutions in BRCA1, implying that the formation of a stable complex between these proteins may be an essential aspect of BRCA1 tumor suppression. This protein may be the target of oncogenic mutations in breast or ovarian cancer. Multiple alternatively spliced transcript variants encoding different isoforms have been found for this gene. In the published literature, this variant has been reported to have proficient homology directed repair (HDR) (PMID: 30925164 (2019)). In a large-scale breast cancer association study, the variant was observed in individuals with breast cancer as well as in an unaffected (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared/genes/BARD1)). The frequency of this variant in the general population, 0.000008 (2/251350 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is deleterious or benign. This sequence change replaces lysine, which is basic and polar, with asparagine, which is neutral and polar, at codon 540 of the BARD1 protein (p.Lys540Asn). This variant is present in population databases (rs747076015, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with BARD1-related conditions. ClinVar contains an entry for this variant (Variation ID: 490942). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change does not substantially affect BARD1 function (PMID: 30925164). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.K540N variant (also known as c.1620A>T), located in coding exon 7 of the BARD1 gene, results from an A to T substitution at nucleotide position 1620. The lysine at codon 540 is replaced by asparagine, an amino acid with similar properties. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

This missense variant replaces lysine with asparagine at codon 540 of the BARD1 protein. Computational prediction suggests that this variant may not impact protein structure and function. A functional study has shown the mutant protein to exhibit ~80% homology-directed repair activity compared to the wild type protein (PMID: 30925164). In a large breast cancer case-control study, this variant was identified in 2/60464 breast cancer cases and 1/53460 controls (PMID: 33471991). This variant has been identified in 2/251350 chromosomes in the general population by the Genome Aggregation Database (gnomAD). Variant summary: BARD1 c.1620A>T (p.Lys540Asn) results in a non-conservative amino acid change located in the Ankyrin repeat-containing domain of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 8e-06 in 251350 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.1620A>T in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome has been reported. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in approximately 80-85% of normal Homology Directed Repair (HDR) activity in vitro (Adamovich_2019). The following publication have been ascertained in the context of this evaluation (PMID: 30925164). ClinVar contains an entry for this variant (Variation ID: 490942). Based on the evidence outlined above, the variant was classified as uncertain significance. -
