When cloned, it was discovered to be a human homolog of the E. coli mismatch repair gene mutS, consistent with the characteristic alterations in microsatellite sequences (RER+ phenotype) found in HNPCC. The histidine at codon 428 is replaced by glutamine, an amino acid with highly similar properties. This alteration was reported in a proband from Iceland diagnosed with colorectal cancer that demonstrated normal staining for the mismatch repair proteins on immunohistochemistry (Haraldsdottir S et al. Nat Commun, 2017 05;8:14755). This alteration was also identified in an individual with colorectal cancer meeting Bethesda or Amsterdam Criteria (Dominguez-Valentin M et al. BMC Med. Genet., 2018 02;19:26).This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
