This variant has been reported in the published literature in an individual with colorectal cancer (PMID: 28944238 (2017)) and an individual undergoing genetic testing (PMID: 28514183 (2017)). This variant has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Based on the available information, this variant is classified as pathogenic. -

The c.1522_1523delAG pathogenic mutation, located in coding exon 13 of the MLH1 gene, results from a deletion of two nucleotides at nucleotide positions 1522 to 1523, causing a translational frameshift with a predicted alternate stop codon (p.L509Pfs*5). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This variant (designated c.1521_1522delGA, Leu509fs) was detected in an individual with colorectal cancer diagnosed prior to age 50 (DeRycke MS et al. Mol Genet Genomic Med, 2017 Sep;5:553-569). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
